To register for
the June Forum, click

You may fill out and submit the form online or you may print a copy of
the form and fax it to Matthew Getz at 319-248-0222.
Registration must
be received no later than Friday, May 18, 2007.
Sleeping Rooms
Sleeping rooms are available
for $109/night. Please call the hotel at (740) 788-1200 to make your
reservation. We recommend that participants stay at the hotel June 21st, 2007.
Indicate you are with the Ohio Leadership Forum when making your room
reservation in order to get the discounted rate.
NOTE: To get the special
sleeping room rate, you must call the hotel direct. While there is no fee for
the program, participants are responsible for making their hotel reservations
and paying for sleeping rooms, parking, and other related travel expenses.
Some scholarships to cover hotel expenses will be
available for superintendents whose districts are in fiscal emergency. If you
would like to inquire about scholarship assistance, contact Matthew Getz (matthew@ohioleaders.org

Meals
All meals are
provided, beginning with breakfast on June 21 through breakfast on June
22. When registering, please indicate which meals you will be present
for. If you indicate you will be present for a meal and your plans
change, please let Matthew Getz (matthew@ohioleaders.org) know by
Friday, June 8, 2007 so we may
eliminate unnecessary expenditures.
Because the June forum is a retreat setting, a few participants are
planning on bringing guests. Therefore, we would like to extend an
invitation to your spouses, children or any other guests you might like
to include to the Reception and Dinner on June 21, 2007 at the Cherry
Valley Lodge starting at 6:00pm on the Lobby Patio. The meal will
be a BBQ dinner. Weather permitting we will have outdoor games at the
BBQ.
Your
own meal is covered by the Ohio Leadership Forum. The price for
the dinner will be $28.95 per additional adult guest and $14.50 per
child payable by participants to the Lodge at the dinner.
Parking
Surface parking at the hotel is
free.
Attire
Business casual
attire is suggested.
